EAGAN HIGH SCHOOL (NON-FEE) FINE ARTS & STUDENT ACTIVITIES AND CURRICULAR ACTIV
LOCAL ACADEMIC AND BEHAVIOR ELIGIBILITY RULES
In grades 9-12 a student must be passing five (5) classes in the previous trimester to be eligible for participation in co-curricular programs at EHS. If the student is not passing (or did not pass) five (5) classes, he/she will be ineligible for a period of time (performances, events) and become eligible again provided that he/she is passing all current classes. In addition, student eligibility may be withdrawn for harassment, bullying, hazing, inappropriate behavior with staff and/or conduct unbecoming a participant.

FINE ARTS AND STUDENT ACTIVITIES CODE
During the calendar year, regardless of the quantity, a student shall not:
• use or possess or distribute a beverage containing alcohol
• use or possess tobacco
• use or consume, have in possession, buy, sell or give away any other controlled substance.
• use or possess drug paraphernalia
Penalties:
First Violation: The student shall lose eligibility for one (1) performance/event/contest in which the student is a participant.
Second Violation: The student shall lose eligibility for two (2) performances/events/contests in which the student is a participant.

All consequences are at the discretion of the principal and the advisor(s).

Coaches, directors and advisors may have expectations over and above these consequences. Students in violation will lose leadership honors and may forfeit awards or letter awards. Students must remain in the activity until it ends in order to complete the penalty. Students may lose membership in special programs according to the program/advisor decision.
Accumulative Penalties: Penalties shall be accumulative beginning with the student’s first participation on a high school team/activity and continuing throughout the student’s high school career. In addition, students involved in both athletic teams and arts/activities will need to serve the consequences in both areas separately.

Student Code of Conduct
As a student participating in my school's co-curricular activities, I understand and accept the following responsibilities:
• I will respect the rights and beliefs of others and will treat others with courtesy and consideration.
• I will be fully responsible for my own actions and the consequences of my actions.
• I will respect the property of others.
• I will respect and obey the rules my school and the laws of my community, state and country.
• I will show respect to those who are responsible for enforcing the rules of my school and the laws of my community, state and country.

Racial/Religious/Sexual Harassment/Violence/Hazing and Bullying
A student shall not violate Racial/Religious/Sexual Harassment/Violence/Hazing and Bullying policy as defined by District 196 and the student handbook.

I have read, understand, and acknowledge receiving the Fine Arts & Student Activities Eligibility information, that I must adhere to all of the rules and regulations, but that local rules may be more stringent. At Eagan High School all performing and competition events/programs/activities are governed by these rules. Please note rules and other information on the back of this form. All EHS activity participation is governed by this agreement (see EHS Student Handbook).

I agree to fully cooperate in any investigation honestly and truthfully. I understand that all decisions are subject to principal and advisor review.

This student/parent authorizes the release of documents and other pertinent information by the school in order to determine student eligibility.

In addition, the student and parent understands and agrees that public information shall include names and pictures of students participating in or attending co-curricular activities, school events, and High School League activities or events. The student/parent(s) understand that all EHS activity participation is included in this agreement.

Parents/Guardians/Adult Students (Age 18 and Older): If your student (or you, if you are a student age 18 or older) plans to participate in cocurricular activities, please complete and sign the following form, and return it to the student’s coach or advisor.

has my permission to participate in co-curricular activities during the 2017-18 school year and I understand that it is my sole responsibility to arrange for my student’s transportation to and from co-curricular events. I understand that Independent School District 196 and Eagan High School may arrange for transportation from time-to-time, but on all other occasions transportation remains my sole responsibility. I agree for myself and on behalf of my student to waive all liability and to release and hold harmless District 196, its School Board members, agents, employees, and volunteers in the event of injury death, loss or damage to person or property that occurs as a result of my student’s transportation to or from a co-curricular activity by transportation other than that provided by District 196. In addition, I agree for myself and on behalf of my student, not to assert against the District, its board members, agents, employees or volunteers, in any court of law, any claim or claims whether known or unknown, that arise as a result of such transportation. I understand that this release and waiver of liability applies to claims caused by District 196’s own negligence, but not by the willful or wanton misconduct of District 196, its board members, agents, employees or volunteers. I have read and voluntarily sign the release and waiver of liability, and further agree that no oral representations, statements or inducements apart from the foregoing written agreement have been made.
